This horse doesn't appear anywhere in-game that I know of, so it appears to be a hidden or unused variant. It looks like an inverse Few Spot Appaloosa - black with white spots instead of white with black spots.

I have seen such a horse in the online story quests. There is a quest where you need to escape to Manzanita Post from Aurora Basin on the back of an escape horse. The cop NPC you are with will take an all black horse with white spots. Very few and far between. I have no idea what this horse is and would like to know
