Limbs have health bars. If an animal falls 3 meters (10 feet) and lands on their legs, they will limp. Your horse smashes into a boulder from a 3 meter (10 foot) drop, it's 4 legs will be crippled and it will move very slowly, if an enemy takes an arrow to the right arm, their accuracy will decrease heavily or they wont be able to damage you effectively with their fists or a melee weapon if both of their arms are crippled. The animation for a crippled leg could be the engulfed in fire animation, or enemies when they've been shot in a leg. The more damage received to an already crippled limb, the more damage that will be received to that limb compared to an un-crippled limb. If your eyes are crippled, your vision will be very blurry. If an arm is crippled, your aim will be unsteady and you will shoot inaccurately. You won't be able to effectively damage an enemy with a melee weapon or your fists if your arms are crippled. If your head is crippled, Dead-eye can't be activated. If both of your legs are crippled, you will limp and if you run/sprint then you will fumble to the ground or move very slowly. If all of your limbs are crippled, then your stamina, Dead-eye, and health cores will only have the cores full while the white bars next to the cores will be empty, your vision will blur and you will pass-out in a few seconds unless you mount your horse. If you pass-out time will progress by a few hours, once you wake-up and if you don't have a horse nearby, it's recommended to call for someone driving a vehicle if you can sit in their vehicle. To heal a (crippled) limb, go to a doctor in Valentine, Saint Denis, Tumbleweed, Armadillo, Rhodes, Annesburg, Central Union Railroad camp, etc. for a fee. Or buy a "doctor's bag" from a doctor to heal a limb when your crippled in the wilderness and currently can't reach a doctor. The "doctor's bag" can also be used on your horse if it's crippled. If your horse has it's legs, torso, and head crippled. It will lay on the ground in a critical state, if you don't have supplies for your horse, then you may have to return to it with a "doctor's bag" after traveling to a doctor to receive supplies. And depending on the caliber of a bullet or ammo type (12 gauge, express ammo, split point, .45, 6.5×52mm) that damaged your character, the power of that weapon will deal more damage to your limbs. bolmin 33
You can check out my injuries overhaul I guess. It's not the same thing you described, but kinda similar in concept.
